Restricted Materials Use form
Some materials in the holdings of the West Virginia & Regional History Center have been identified as restricted and require special permission for use. These collections may contain personally identifiable information, medical and educational records, and/or sensitive materials. The WVRHC seeks to balance making information available to researchers with protecting the privacy of individuals and upholding laws that restrict access to personal information.
To use these materials, researchers must follow the rules outlined below:
- No reproductions created by scanning, photography, photocopying or any other means are permitted.
- Data culled from materials containing personal information may only be used in statistical aggregate or as an example to illustrate a theme. Information identifying individuals, families, or organizations should not be recorded, copied, or published.
- Publication of direct quotations, entire passages or complete forms is prohibited.
- The researcher is liable for any consequences resulting from the publication of personal information in their work.
- The researcher also agrees to be bound by any terms and conditions which may affect their use of materials as contained in the WVRHC Access Policy, and any other West Virginia University policies and procedures governing collections.
